NarrativeAt 9:00 a.m. on November 30, 2017, an employee was working for a wholesaler of grain and field beans. The facility had a Hayssen filling, bagging, or sealing machine. The machine was running on the facilitys Line Number 1. The employee was clearing plastic bags from the blade on the machine. Without shutting the machine down, she reached under the machines guard to retrieve plastic bags. She sustained an amputation of the fingertip of the index finger on her left hand. She was not hospitalized.
