NarrativeAt 10:30 a.m. on September 28, 2017, Employee #1 was operating a powered reach truck. There was a spill of hydraulic fluid on the floor causing the truck to slide into pallets of empty kegs. Two of the empty kegs fell from the top row, crushing the employee's hand. Employee #1 sustained a broken hand and fingers. He was hospitalized for treatment.
The accident investigation below was opened on Tuesday, October 31st 2017 by OSHA representitives in Los Angeles, CA which had occured previously at Ace Beverage Llc. A 62 year old had been injured when the employee was struck by forklift and crushes both legs

NarrativeAt 5:30 p.m. on August 22, 2017, an employee was struck by a forklift that was being operated by a coworker who was off-loading pallets of alcoholic beverages. Both of the employee's legs were struck by the forklift and crushed as the coworker backed the vehicle up. The employee was hospitalized with multiple fractures of both legs.
On Thursday, June 22nd 2017 OSHA reps took a report of a serious accident in Auburndale, FL at Caribbean Distillers Llc. A 33 year old man working as a Communications equipment operator was killed on the job when the employee contacted exposed wire and was electrocuted

NarrativeAt 4:55 p.m. on June 21, 2017, Employee #1, a power communications line technician, was servicing the electrical lines of a conveyor belt. As he was removing the insulation from a 270 Volt wire, his hand contacted the exposed wire and he was electrocuted.
