NarrativeAt 3:00 p.m. on January 13, 2018, an employee was assigned to clean the exterior of a tanker trailer and a container it was hauling. After washing the exterior of a tanker trailer, the employee entered the container and was overcome by its vapors. The employee was found unresponsive inside the container.

NarrativeAt 5:50 p.m. on July 10, 2017, an employee was attempting to remove a spike from underneath a conveyor. The spike was under the conveyor near an unguarded belt and pulley. The employee's hand was pulled into the belt and pulley and his right arm was amputated at or near the shoulder.
