NarrativeAt 7:45 a.m. on December 6, 2018, an employee was repairing a hydraulic leak on a John Deer Front end loader. The employee was working under the raised bucket of the front end loader when he drained the hydraulic fluid, causing the bucket to fall and crush the employee. The employee was killed.

NarrativeAt 11:00 a.m. on October 16, 2017, Employee #1 and a coworker were stacking lumber on a conveyor. During work, the conveyor motor stalled. Employee #1 climbed under the conveyor on the stacker/trimmer to jump start the motor by hand. The employee told the coworker to shut down the conveyor, then restart on his mark. Employee #1 instructed the coworker to restart the conveyor while he had his hand on the pulley to assist it, but it did not start. After a second attempt, the motor started and pulled Employee #1's hand into the pulley, amputating his finger.

NarrativeAt 10:00 a.m. on September 26, 2017, an employee was ripping 2-inch strips of plywood from a full sheet of 0.5 inch OSB Plywood. The employee 's hand slipped and struck the saw blade while pushing the wood through the saw. The employee amputated his finger. The employee was hospitalized and treated for his injuries.
