NarrativeAt 10:15 a.m. on September 5, 2017, an employee was removing the insulated blankets while standing inside a bucket truck basket working on replacing cut outs to an energized electrical power line at the pole area. The employee contacted the cut out equipment with his chest and was electrocuted.

NarrativeAt 4:22 p.m. on June 29, 2017, an employee was in the process of trying to unplug a hole in a molten slag tank. An explosion occurred releasing molten slag and the employee burned upon contact with material. Two employees were killed, and four employees were hospitalized. No other information was given about the employees who were hospitalized.

NarrativeAt 1:30 p.m. on June 21, 2017, an employee was installing temporary wiring at a new construction site and was working from an extension ladder. The employee was leaning against a 6 inch by 6 inch post and was pulling the wire to secure it to the post. The employee fell from the ladder when the post snapped and suffered a broken ankle. The employee was transported to the hospital and was treated for his injuries. The employee was hospitalized.
