NarrativeAt 4:00 p.m. on September 12, 2017, an employee was working for a marine cargo terminal. He was adjusting a twist lock on a shipping container, when the crane operator lowered a container onto his left hand. The thumb and portions of his index and middle fingers on his left hand were amputated, and the employee was hospitalized.
The accident investigation below was opened on Monday, March 21st 2016 by OSHA representitives in Paducah, KY after learning of an incident at Western Rivers Boat Management which had occured on Friday, March 18th 2016. NarrativeAt 11:30 a.m. on August 27, 2012, an employee arrived at the Trailer Control area at the CPRS Isla Grande Terminal to deliver an empty container. The employee exited his truck and was standing near the aft section of the rear axle wheels when he was struck by a second truck that came around the passenger side of his vehicle, killing him on-site.
