NarrativeAt 9:10 a.m. on November 7, 2017, Employee #1, a supervisor, and a coworker were examining an energized 480 volt breaker panel that contained a breaker that would not turn on. The panel had 2 round screw cover plates that covered the breakers. The coworker unscrewed the bottom plate and observed two 3 phase breakers, one on top of the other. The coworker also observed that the breakers were just "push in" type breakers. The breaker that was not functioning was the top breaker. The top of the top breaker was behind the fixed panel cover and to pull the top breaker out, the bottom breaker had to be removed first. Without deenergizing the panel, Employee #1 pulled the bottom breaker out and began pulling the top breaker out. An arc flash occurred, causing Employee #1 burns to his face, chest, arms ,and hands.
On Tuesday, October 3rd 2017 OSHA reps took a report of a serious accident in Columbus, OH after learning of an incident at Unique Stone Concepts, Llc. which had occured on the same day. NarrativeAt 11:15 a.m. on October 3, 2017, an employee was operating a small mobile crane in the storage area of the loading docks. The employee was hoisting a bundle of granite slabs from the transfer A-frame rack to the storage rack. The employee was working alone. The employee placed the bundle, approximately six-thousand pounds, on the rack. The employee exited the cab of the crane and approached the storage rack. The employee reached in between a previously stored bundle and the bundle placed by the employee in order to set a rack post for the bundle to lean against. As the employee was in between the bundles, the bundle set shifted and crushed the employee. The employee was crushed at the head/shoulder/chest area. His coworkers removed the employee from the storage rack and performed CPR until EMS arrived. The employee was transported to the hospital. The employee died as a result of the crushing injuries sustained.

NarrativeAt 11:00 p.m. on September 22, 2017, Employee #1 was working at a storage yard (around 3 acres) associated with the export of salvaged vehicles from California to a wrecking yard in Tijuana, Mexico. Employee #1 was unloading 4 vehicles off a 4-car hauler transport flat trailer (4NW1551) at the yard. Employee #1 was unloading the last vehicle, a manual 1994 GMC Sonoma truck. The vehicle was in the neutral position with no emergency brake on. The truck slide off the ramp sideways when the chains were removed and Employee #1 was unable to get out of the way and was pinned underneath it. The employee died on site from mechanical asphyxia and blunt force injury of chest.
