NarrativeOn June 22, 2012, an electrical worker installing a service box at the gable of a building approximately 24 feet above the ground was using a sissor platform to gain access that did not reach. The employee then attempted to use a loader equipped with forks to raise the sissor platform even higher. However, the sissor platform fell from the forks on the loader ejecting the worker to a concrete pad. The employee struck his head on the concrete. The employee was killed.
