NarrativeOn June 29, 2012, Employee #1 was operating a forklift and unloading a box scrapper off a flatbed delivery truck. As he moved the box, it became stuck on one of the forks of the forklift. He pulled on the box to free it. It fell from the forks, landed on his right foot and crushed the big toe. A coworker transported Employee #1 to a medical center. At the hospital, he underwent treatment and surgical procedures that involved amputating the right big toe. Employee #1 was hospitalized for postoperative care.
