NarrativeAt 4:00 p.m. on November 7, 2018, Employee #1 and Employee #2 were called out on a roadside repair job on a tractor trailer tire. Upon completing the job, Employee#1 crawled underneath the trailer at the same time as the driver started to drive away running the employee over. The driver started to pull away at the exact same time Employee #1 went underneath the trailer for an unknown reason and was caught by the tires of the trailer. The tires rolled over the chest and head of the employee. Employee #1 was killed from crushing head and upper body injuries.

NarrativeAt 10:30 a.m. on February 6, 2017, an employee was working at a vehicle repair shop. While descending a ladder, carrying a steel wheel that was located in an elevated storage area, the employee tripped and fell. He struck his head on the shop floor. The employee was hospitalized and treated for head trauma that caused brain swelling.
