On Wednesday, July 19th 2017 OSHA reps took a report of a serious accident in Vega Baja, PR after learning of an incident at Comercial Tortuguero Inc. which had occured on the same day. NarrativeAt 8:00 a.m. on July 19, 2017, an employee was conducting painting activity. There was a work basket being lifted by a forklift just above where the employee was standing. For an unknown reason, the basket dropped and struck the employee in the head. The employee was killed as a result of his injury.

OSHA investigators arrived in TEMPLE, TE on Friday, September 30th 2016 and began an accident investigation which had occured previously at Harbor Freight Tools. A had been injured when the fall on same level, n.e.c.
