NarrativeAt approximately 11:15 a.m. on April 15, 2016, Employee #1 was performing repair work on an elevator. He was helping a coworker replace saddle bolts in an elevator. While Employee #1 was working, he contacted an electrified motor controller panel and was electrocuted.
OSHA investigators arrived in 12th Fl., Ne on Wednesday, April 1st 2015 and began an accident investigation which had occured previously at Nycha Albany Houses. A 59 year old man working as a Painter was killed on the job when the employee died from heart attack.

NarrativeAt 1:00 p.m. on March 31, 2015, an employee was spraying sheetrock texture on the ceiling, when the employee passed out on his way to the shop, outside of the building. The employee died from cardio-vascular disease.

NarrativeAt 1:45 p.m. on March 7, 2015, a supervisor of an garbage collection employee reported a missing employee incident to the Emergency Service Unit. The supervisor performed a search operation at the building and found the garbage collection employee near the refuse hoist located at the ground floor. The employee was found unconscious and her body parts were trapped in the refuse hoist located at the ground level. The emergency medical services responded and removed the employee and transported her to hospital and at approximately 2:30 pm, she was declared dead on arrival at the hospital.
