NarrativeAt approximately 11:30 a.m. on October 20, 2011, Employee #1, of Belfor Property Restoration, was using a Ridgid 12 in. compound miter saw (Model Number MS150LZ and Serial Number U034736675) to cut 2 in. by 6 in. by 8 ft long boards. Employee #1 was cutting the board and pushed it over too hard. The board and his hand hit the blade. Employee #1 was transported to UCSD at Hillcrest, where surgery was performed reattaching his left thumb and index finger. The resection of Employee #1's index finger was not successful, and subsequently had to be permanently amputated.
