NarrativeAt 3:30 p.m. on December 7, 2017, an employee was working with a coworker to remove a broken excavator track pin. The employee was struck in his left eye with a shard from the pin when the employee's coworker struck it with a steel hammer. The employee was hospitalized.

NarrativeAt approximately 8:30 a.m. on February 24, 2014, Employee #1 was removing a drain plug from an isolated asphalt oil line at an unloading pump. He utilized a pipe wrench to loosen the drain plug. With his gloved left hand, he started removing the plug from the line. Once the plug was removed, hot residual oil ejected from the hole and struck his left forearm between his glove and elbow. The hot asphalt instantly burned his skin. Employee #1 was transported to a medical center, where he was treated for first and second degree burns and released the same day.

NarrativeAt approximately 1:00 a.m. on September 23, 2013, Employee #1 was operating a wheel loader with an open cab. He loaded a batch of clinker, a caustic cement mixture, into a feed hopper and then a pile of hot clinker behind the loader sloughed. When hot clinker batch came in contact with the clinker in the feed hopper, it created a cloud of dust that engulfed the loader and himself. He was exposed to the caustic dust, causing serious burns to the his extremities, head and neck. Employee #1 was transported to a medical center, where he was hospitalized and treated for multiple burns.
