NarrativeAt approximately 5:45 p.m. on June 17, 2017, Employee #1 was servicing a CAT 980K Front-End Loader (Equipment Number 9812) and removing a liquid-ballast adapter. The liquid-ballast adapter was filled with scalding hot water. As he worked on the liquid-ballast adapter, scalding hot water was ejected from the adapter. The scalding hot water struck his upper legs, groin and lower stomach and caused second and third degree burns on the body parts. Employee #1 was transported to a medical center, where he was underwent burn treatment and was then hospitalized.

NarrativeAt 8:00 a.m. on May 30, 2017 an employee was climbing off the earth moving scrapper mining equipment using the equipment steps after greasing the equipment. The employee lost his balance and fell over six feet to the ground. The steps that on the equipment were small and measured about 6 inches by 6 inches. The employee sustained a broken left ankle which required surgery.

NarrativeAt 1:30 p.m. on April 24, 2017, two employees were performing a rubber asphalt 109 test for Caltrans.While pumping oil into a test tank, both employees suffered burns of varying severity on their hands when there was an overspill of oil and tar. Both employees were hospitalized and treated for their injuries.
