NarrativeAt 10:00 a.m. on November1, 2018, an employee was hitting on some boards that were jammed between the picket and an inclined conveyor belt with a hammer. The employee pinched his fingertip when a chain moved and caught his gloved left hand, amputating it.

NarrativeAt approximately 11:35 a.m. on October 5, 2017, an employee was working alone to replace a loose key stock on the Sweed C-Turner. The employee was positioned inside of the Sweed C-Turner to gain access to the key stock location. During the key stock replacement, the C-Frame rotated, pinning the employee between the C-Frame and rotating base frame. The employee received lacerations and crushing injuries to multiple parts of the body between the shoulders and hips. The employee was killed.

NarrativeAt 3:15 p.m. on August 27, 2017, an employee was working between the stop plate of a tram car and a treatment cylinder. The employee was struck by a tram cart that was driven by a coworker. The employee was crushed between the cart's stop plate and the back wall of the cylinder, killing him.
