NarrativeOn February 27, 2019, an employee accessed his truck (model number: 9200i) and trailer (model number: DGTC-45) that had been loaded with lumber the day before. After checking that the load was secure, he drove to the scheduled delivery site. The employee maneuvered the truck and trailer to an unloading area where he was met by a forklift driver. The employee exited the cab and used a pry bar to loosen the ratchet straps that secured the load and then threw the hooked end of the straps across the load. One of the straps, which was closest to the trucks cab became hung up on the lumber. As the employee climbed onto the trailer and worked to free the stuck strap, he lost his balance when the strap was dislodged. The employee fell backwards and fell from the truck, landing on his feet at first and then collapsing. He struck his head on the asphalt. Emergency services were called; however, the employee died from his injuries.
On Wednesday, January 17th 2018 OSHA reps took a report of a serious accident in Rancho Cucamonga, CA at American Nonwovens, Inc.. A 47 year old man working as a Machine operator was injured on the job when the employee amputated right arm and hand in machine's nip point

NarrativeAt 8:45 a.m. on January 3, 2018, an employee was cleaning a roller on the LM4 machine with a brush. The employee had his hand pulled into a pinch point when the brush was caught by the roller and the employee degloved the lower half of his right arm and hand. The employee was hospitalized.
On Tuesday, September 20th 2016 OSHA reps took a report of a serious accident in Fairburn, GA which had occured previously at Estex Fabric Fabrications, Inc. A had been injured when the caught in running equipment or machinery during regular operation

NarrativeThe employee was operating a grommet machine placing the bottom and top grommet on the canvas materials when she noticed the top grommet was not straight. She tried to straighten it and the hammer came down on her right middle finger, crushing the fingertip.
