NarrativeAt 12:45 p.m. on November 7, 2017, an employee was driving a clamp truck up a loading ramp to enter a building. He pulled the truck to the far side of the curb with one of its wheels hanging off the curb. The truck began to tilt and became unstable when the employee raised the truck clamp into the air to the full height. The truck began to fall over. To avoid being trapped in the cab of the truck, the employee jumped from the truck. However, as the truck fell to its side, the employee slipped under the truck and was struck by the truck's cage. The employees back and lower extremities were injured. He was hospitalized and received treatment for his injuries.
On Tuesday, October 3rd 2017 OSHA reps took a report of a serious accident in Dalton, GA which had occured previously at Shaw Industries Group, Inc. . A 56 year old man working as a Miscellaneous machine operator was injured on the job when the employee catches finger in conveyor belt and amputated it

NarrativeAt 1:15 p.m. on September 29, 2017, an employee was working at the Mega Cutter Line which was located between two Tile Turning Conveyors. The employee was clearing a tile back up in the beveling area by placing the the tiles on the floor. The employee fell forward after tripping on the tiles he just placed on the floor. As the employee attempted to catch himself, his left index finger was caught in conveyor belt that turns the tiles around for side trimming and amputated his middle finger. The employee was hospitalized.
OSHA investigators arrived in PONTOTOC, MI on Wednesday, September 21st 2016 and began an accident investigation which had occured previously at Fibrix. A had been injured when the caught in running equipment or machinery during maintenance, cleaning
