NarrativeOn November 9, 2016, Employee #1, a card technician, was working on card machin e number 14. The licker-in mechanism came up out of the card machine and struc k the cover and then the cover and the licker-in struck him. He sustained skul l, rib, and arm fractures and an arm laceration. He was hospitalized due to in ternal injuries and later died.

NarrativeWith the assistance of other employees, the injured employee was relocating a tufting machine. The employees were in the process of leveling the machine by inserting metal leveling plates under the machine. The machine was being lifted by a 10-ton capacity pneumatic floor jack. The jack slipped and the tufting machine dropped trapping the employee's hand underneath. The employee suffered two fractured fingers.
