NarrativeAt 10:30 p.m. on May 24, 2018, an employee was operating a steel tube End-Forming machine and removing a dimple from the inside diameter of an approximately 1 1/2 inch diameter pipe. The employee was talking with a coworker and leaned against the machine when he stepped back onto the foot peddle and cycled the machine. The employee caught his finger between the guard and a loose adjustment nut, amputating his fingertip. The employee was hospitalized.
On Tuesday, December 12th 2017 OSHA reps took a report of a serious accident in Piqua, OH which had occured previously at Jackson Tube Service, Inc.. A 23 year old Machine operator had been injured when the employee's finger was amputated

NarrativeAt 5:00 a.m. on December 5,2017, Employee #1 was removing steel tubes from the dump table on a mill.One of the steel tubes got hung up on the brass vee roll and Employee #1 reached in to pull it out and got her finger caught between the brass vee roll and the frame of the table, as the brass vee roll was closing. Employee #1 finger was amputated.

NarrativeAt 3:15 p.m. on October 25, 2017, an employee was working for a firm that manufactured iron and steel pipe from purchased steel. He was rolling a large steel pipe on a pipe rack. The employee's ring finger and middle finger on his right hand became pinched between two steel pipes. He sustained the avulsion of his finger nail and a laceration of his right middle finger. The tip of his right ring finger was partially amputated, and it had an open fracture. He was hospitalized.
