NarrativeAt 10:00 a.m. on July 20, 2017, an employee was cleaning the Teflon guide roller. The employee was pulled into the machine and suffered an arm fracture as well as third degree burns to their neck, shoulder, arm, and legs.

NarrativeAt 8:45 p.m. on February 3, 2017, an employee was positioning asphalt sheet feed material into rollers by hand. The employee hit the power button with his hand on the materials to feed up through roller. Without adequate support from fellow employees to help with weight of roll material, the employee got caught in the unguarded machine. The employee sustained ligament damage and strain on wrist tendons and hand.

NarrativeAn employee was beginning to install a thermocouple. He pulled out a plug and discovered it was not connected to a thermowell, but was plugged directly into the asphalt storage tank. When the plug was removed, 295 ?F asphalt splashed out of the tank and onto his hands, upper chest, and neck. He received second degree, partial thickness burns.
