NarrativeAt 6:00 a.m. on April 4, 2019, an employee died from abdominal injuries. The employee initially sustained his injuries on March 21, 2019, when he stepped on a drag chain conveyor pan to dislodge a stick and became caught. The employee was pulled into the equipment's tall spool by his leg. His leg was amputated and he also sustained abdominal injuries.

NarrativeAt 12:30 p.m. on August 27, 2017, an employee was attempting to mount an aerial lift while it was descending from a third floor level. The employee fell from the lift and struck his head and body on the ground. The employee was killed from blunt force trauma injuries.

NarrativeAt approximately 3:10 p.m. on October 6, 2016, an employee was sitting down on a metal bleacher step in front of Line No. 1 CD Filter. There were eight manhat ches along Line No 1 CD Filter pressure vessel. The third door, which measured 24-in. diameter, from the right side blew out. The employee was struck and was found on the floor below.
