NarrativeWorker had just picked up a load of chemicals for the supplier and he was removing the wheel chocks from the trucks wheels. He had removed the first set of wheel chocks and moved to the second set. As he pulled them free from the wheels, he tripped over the first set he had pulled. He fell to the ground and landed on the second set of chocks. He broke his hip and possibly injured his back.
OSHA investigators arrived in Kinston, NC on Wednesday, December 23rd 2009 and began an accident investigation which had occured previously at Lenox China Manufacturing. A employee working as a employee was killed on the job when the employee died from heart attack

NarrativeOn December 22, 2009, Employee #1 was repairing a machine, when he fell over bac kwards. Cardiopulmonary resuscitation was begun immediately, and he was taken to hospital, where he was pronounced dead. The cause of death was determined to be a heart attack.
