OSHA investigators arrived in Elgin, IL on Wednesday, August 16th 2017 and began an accident investigation after learning of an incident at Concrete Specialties Co. NarrativeAt approximately 8:15 a.m. on August 10, 2017, a maintenance employee was making an adjustment to a conveyor belt for a line that transports loads of wet concrete that the company referred to as the Big Machine Mud Line Belt. The belt was on while making the adjustment as that was the standard protocol. The employee's thumb was pulled into a belt and pulley drive while the conveyor was running, nearly severing the right thumb. There were no direct witnesses to the accident. The employee was hospitalized and treated for his injuries.
