NarrativeAn employee had his hand on a screen printer while another employee was performing a calibration. The machine went to park and caught the employee's left thumb. The left thumb was amputated from the nail forward.
OSHA investigators arrived in Atglen, PA on Saturday, March 21st 2015 and began an accident investigation after learning of an incident at Quad Graphics, Llc which had occured on Friday, March 20th 2015. A 44 year old had been killed when the employee killed when caught in a paper baler

NarrativeAt 6:00 p.m. on March 20, 2015, an employee was clearing a jam from the paper f luffer while standing in a Fluffer Baler chute. The employee was killed when en gulfed by falling paper that broke free from the jammed paper fluffer baler.

Narrative An employee sustained an injury to his left pinky finger as he was polishing a coil with a CNC lathe. The door safety interlocks were disengaged allowing the employee to polish parts of the coil as the machine was running.
