NarrativeAt approximately 1:15 p.m. on February 10, 2017, Employee #1 was observing another maintenance employee adjust components within a horizontal boring machine. Employee #1 decided to check the tension in the machines pulley and belt system and placed his right hand into the machine. The maintenance coworker then started the machine. Employee #1s right hand was caught by the pulley and belt system and pulled into the machine, which fractured and lacerated it. Employee #1 was transported to a medical center, where he underwent surgical procedures, which included the placement of pins to repair the hand. He also received post-operative treatment and care, and was then hospitalized.
On Friday, April 15th 2016 OSHA reps took a report of a serious accident in CENTRALIA, IL at AMERICAN EQUIPMENT AND MACHINE, INC.. A had been injured when the compressed or pinched by shifting objected or equipment

NarrativeAn employee was adjusting the chains on a piece of equipment, preparing it to be lifted by an overhead crane. The coworker operating the crane let go of the control button for lowering the chains. The injured employee thought the crane had already come to a complete stop, so he attempted to adjust the chains. His left hand was on one of the chains and while adjusting the other chain with his right hand, the chain tightened and slapped against his left hand, amputating his left middle fingertip.
On Saturday, December 5th 2015 OSHA reps took a report of a serious accident in HOMER CITY, PE after learning of an incident at Joy Global which had occured on the same day. A had been injured when the struck by swinging or slipping object, other than handheld, n.e.c.

NarrativeAn employee suffered a serious injury to his right hand while positioning a 1700 pound could blade onto a table. The injured employee suffered a broken index finger and cuts to three other fingers on his right hand when the part shifted as it was being moved with an overhead crane. The injury occurred in the weld shop.
