NarrativeAt 12:00 p.m. on October 17, 2017, an employee was helping unload a folding cart used to prepare show sheep. The cart needed repair. The employee was moving the folding cart and caught a finger in a pinch point on the cart. The employee's finger was partially amputated. The employee underwent surgery to reattach tendons and a repair the bone.
OSHA investigators arrived in Washington, IA on Wednesday, December 20th 2017 and began an accident investigation which had occured previously at Bazooka Farmstar Llc. A 28 year old Production helper had been injured when the employee was injured when struck by 720 pound hose hustler

NarrativeAt 5:15 p.m. on October 16, 2017, Employee #1 was prepping a 720 pound hose hustler for paint. After prepping one side of the hose hustler, Employee #1 removed the hose hustler from its stand and spun it around to access the other side. As he spun the hose hustler, it fell and struck Employee #1's left chest and right femur. He was hospitalized overnight for observation.
