NarrativeAt 1:00 p.m. on April 23, 2018, an employee was cleaning the buildup fiber on the carding machine cylinders while it was operating and a coworker increased the machine's processing speed. The employee had their right index finger pulled into the machine's cylinders and amputated it.
OSHA investigators arrived in Orleans, VT on Thursday, December 14th 2017 and began an accident investigation at Ethan Allen Operations, Inc. A 55 year old man working as a Machinery maintenance occupation was killed on the job when the employee was caught in silo auger and was killed

NarrativeAt 3:35 p.m. on December 13, 2017, an employee entered Silo #3 to check on the performance of a repair bottom unloader auger. The auger had been doing a live run in test and the employee was caught by the auger. The employee had both legs and right arm ground off by the automatic action of the auger and suffered lacerations to his torso while trying to free himself. The employee was killed at the scene.

NarrativeAt 3:02 a.m. on October 23, 2017, an employee was positioning a pallet jack to pick up an empty pallet.As the employee was positioning the pallet jack, it started spinning around until it struck a railing. The employee's left ankle was caught between the pallet jack and railing, resulting in a fractured ankle.
