Fruit and Vegetable Canning

NarrativeAt 9:30 a.m. on November 7, 2017, an employee was straightening cans and inserting cardboard between rows of cans in a Whallon 300 Brite Stacker Palletizer. The employee walked around the machine guard to put a can in an empty spot on a previously placed layer of cans. Resting his left hand on a layer of cans for support, he used his right hand to place a can in the empty spot. The machine cycled down on top of his left hand, amputating his left middle finger tip.

NarrativeAt 9:40 a.m. on October 9, 2017, an employee was cleaning a work area while waiting for maintenance repairs to be completed on a waste removal system. The employee noticed that there was a corn cob caught inside the Nubbin Eliminator Belt and attempted to remove it by reaching in with a metal rod from the south side. The metal rod was a 1/4 inch rod bent at one end into a handle and the other end was shaped into a makeshift hook. The employee was pulled into the in-running nip point created by the bottom belt and the tail pulley and suffered an amputation to the right arm above the elbow. The employee was hospitalized.

NarrativeAt 7:15 a.m. on September 25, 2017, Employee #1 was getting reports and crossed the warehouse floor in the path of a forklift. The forklift operator did not see the employee, and struck and dragged her approximately 10 feet under the forklift. Employee #1 sustained a crushed leg and ankle and was hospitalized for her injuries.
