NarrativeAt approximately 3:00 p.m. on June 24, 2017, an employee attempted to reinstall the conveyor belt that had slipped off its tracks. During this process, the employee's finger became caught in the pinch point between the moving belt and the roller, causing an amputation to the tip, right ring finger. The employee was not hospitalized.

NarrativeAt approximately 10:45 a.m. on February 3, 2010, Employee #1 was assisting in th e removal of large cheese vats from the facility. One vat swung as it was being moved by a forklift and struck the side wall of the opening it was being carried through. Employee #1 was struck by the swinging vat and sustained a fractured a nkle. Employee #1 was hospitalized.
