NarrativeAt 9:30 a.m. on September 21, 2018, five employees from two different companies were loading large machine shop equipment onto a flat bed truck during the liquidation of one of the companies. While loading a large tube bender on the truck using a forklift, an unsecured mechanical press brake, previously loaded onto the truck, listed and fell from the truck bed and landed onto one of the employees. The employee sustained fatal blunt force trauma / crushing injuries with the cause of death listed as exsanguination.

NarrativeAt 8:30 a.m. on November 15, 2017, an employee was assisting a vendor with the setup of a new machine (KBP Roll Forming Machine; serial #2177014). The employee was not familiar with this machine and the machine was not guarded. The employee's hand was crushed when the rack and pinion moved, amputating his finger.

NarrativeAt 6:15 a.m. on November 1, 2017, an employee was feeding a piece into the roll former machine. The employee attempted to pull out the piece that was becoming deformed, resulting in the amputation of their left index finger.
