NarrativeAt 10:00 p.m. on November 12, 2018, an employee was using a rag to clean a roll-coater machine. During cleaning, the rag became caught in the machine's rollers. The employee's right hand was pulled in the rollers, resulting in the amputation of her index finger.

NarrativeAt 7:30 a.m. on January 2, 2018, an employee was working as a yard worker for a truss manufacturer. He was working on the unload side of a purlin machine. His coworker, the operator of the purlin machine, was on the load side. The two were running pieces of wood 4.9 meters 16 feet) long through the purlin machine. The pieces of wood were 38 by 89 millimeters (nominally, 2 inches by 4 inches) in dimension. The employee reached his hand near the hydraulic-powered plate positioner or carriage to unjam a board. He sustained the amputation of the ring finger on his left hand. He was not hospitalized.

NarrativeAt 5:30 on October 16, 2017, Employee #1 was using an overhead crane to maneuver a 1,250 pound bundle of metal roof panel sheets that were stacked and bound onto a transport trailer. As Employee #1 used the crane's hand controls, walking beside the bundle being moved, the load slipped from the chain hooks and struck Employee #1's leg. Employee #1 became trapped beneath the load, sustaining a broken leg and was hospitalized for his injury.
