NarrativeOn Sunday June 7, 2015, at approximately 4:55 p.m., an employee on the production floor was operating a crimper machine, which is a wire connector crimping machine operated by an unguarded foot pedal. The injured employee was hand-feeding wire pieces into the point of operation when she inadvertently pressed a foot pedal with her right-hand index finger, resulting in an amputation. The point of operation was unguarded at the time.

NarrativeAn employee was working on the buffering lines running a cable. The jacket came off while he was trying to restring the machine. He pulled the yarn and the dance arm became loose pulling the yarn and causing it to tighten. The yarn tightened around his right middle finger causing an above the bone amputation.
