NarrativeAt 10:40 a.m. on October 24, 2017, an employee was pushing a motor down a conveyor after testing the brake. The test equipment failed to stop the motor from rotating, and when the employee reached to push the motor, he was struck by the shaft on his ring finger. The employee's finger was later amputated at the second knuckle. Hospitalization was not required.

NarrativeAt 4:15 p.m. on December 7, 2016, an employee finished adjusting a generator and had phoned his manager. The employee got out of his truck. The truck rolled down the hill striking and killing the employee.The parking break for the truck was not set, and chock blocks were not used.

NarrativeAn employee was changing a roll on a packaging machine when a roll of paper became hung up on a frame. The employee attempted to free it by pushing on the roll. When it came free, the employee's right index finger was caught between the frame and the roll of paper, amputating it at the first digit just behind the nail.
