NarrativeAt 11:10 a.m. on August 30, 2018, an employee was performing a clean out/blow down at the Line 1 rotary airlock. During the clean out/blow down the employee started and stopped the power on the rotary airlock to perform cleaning. The power to the rotary airlock was "on" during clean out/blow down of the rotary airlock. The employee sustained amputations to his fingers.

NarrativeAt 12:55 a.m. on January 22, 2017, an employee was conducting repair related ac tivities on the upper level of a spin dryer on Line 3. The employee climbed ou t and was standing on the cover of an auger/screw conveyor system when the empl oyees foot slipped and they fell into the conveyor. The employee fractured hi s leg and lacerated his heel. The employee was hospitalized and treated for hi s injuries.
