NarrativeAt 5:30 a.m. on October 10, 2017, an employee was using a water hose to melt frozen ice at the base of an ice maker. The employee was pulled into the auger when the hose became entangled in the auger. The employee suffered multiple lacerations and fractures. The employee was hospitalized.

NarrativeAt 5:00 p.m. on September 26, 2017, Employee #1 was operating a forklift, moving a block of ice to be placed in a truck. The employee lost consciousness and drove over the four foot dock, with the forklift still in operation. The forklift rolled over onto the employee. Employee #1 died at the scene from either natural causes or impact of the forklift.
