NarrativeOn December 5, 2012, Employee #1 of Three Dots was injured. Employee #1, who works in the employer's Cutting Department, returned to his station after morning break and turned a Blue Streak cutting machine on to resume cutting. However, he was distracted, momentarily looked away, and his left hand struck the cutting blade. The right side of the fleshy tip of his finger was cut away by the blade. Employee #1 was taken to a local clinic by another employee, and from there he was transferred by emergency responders to a local hospital, where he was treated for his injury.
The accident investigation below was opened on Monday, October 29th 2012 by OSHA representitives in Irvine, CA after learning of an incident at Gsm Retail which had occured on Wednesday, September 26th 2012. NarrativeOn September 26, 2012, an Employee #1 was working as a warehouse tagger for a manufacturer of casual clothing. Employee #1 was a direct hire of the employer. She was pulling orders from warehouse racks and stacking them on a hand truck. A coworker, who was also pulling orders, retrieved a forklift and was backing it up in the same aisle in which Employee #1 was working. He backed the forklift to the right, behind a pallet stacked high with goods, and struck Employee #1, who was pulling orders directly behind the pallet. Her hand truck partly shielded her from the forklift, but she was momentarily pinned between the forklift and a warehouse rack upright before the coworker saw her and pulled the forklift forward. Employee #1 sustained a fractured pelvis, five fractured ribs, and two fractured vertebrae. She was hospitalized for treatment of her injuries. The incident was reported later the same day.
On Thursday, November 3rd 2011 OSHA reps took a report of a serious accident in Los Angeles, CA which had occured previously at American Apparel (Usa) Llc. A employee working as a Textile sewing machine operator was injured on the job when the employee's finger was lacerated while hanging clothes

NarrativeAt approximately 2:00 p.m. on October 21, 2011, Employee #1, a sewing machine operator with American Apparel USA, LLC, sustained a laceration to her right ring finger from a metal clothes hanger while repositioning and hanging garments on a portable clothing rack. Two days later, she was admitted to Huntington Hospital for treatment of the laceration and an infection.
