NarrativeAt 8:00 a.m. on September 22, 2017, Employee #1 was cutting wooden board into strips, using a Powermatic 66 table saw. The board became pinched between the blade and the fence. The employee reached over the saw blade to the other side of the board, just as the the blade jumped. Employee #1 sustained an amputation to the distal tip of his right thumb.

NarrativeAt approximately 2:20 p.m. on September 7, 2010, Employee #1 was working in a pr inting press area operating a 5-color lithographic printing press. He was in the process of setting up Press Number 2 and was running it in a slower speed than full production speed. He was making adjustments to the color keys when his righ t index finger was caught in the inbound rollers. Employee #1's right index fing er was lacerated.
