NarrativeAt 10:00 p.m. on January 5, 2019, Employee #1, a CNA was attending to a resident with the assistance of Employee #2 who is also a CNA. Employee #1 was changing the adult underclothes of the resident which were soiled. Employee #2 was keeping the resident calm so that Employee #1 could attend to the resident's personal hygiene. The resident gouged Employee #1 with her fingernails leaving a "seven" or "v" shaped wound which was bleeding. The Employee #1 was admitted to the hospital on 1/9/2019 for septic shock. Group A streptococcus was found in the right forearm lesion.

NarrativeAt 5:30 p.m. on December 21, 2017, an employee was carrying an 8 lb bag of personal items to her car when she slipped and fell on an icy sidewalk. The employee was discovered on the sidewalk by coworkers, bleeding from head and vomiting. The employee was transported to hospital where she was treated for a non-displaced skull fracture. The employee was later transferred to a rehab facility where she died 26 days after the initial incident from complications that resulted from a blunt force head injury.
