NarrativeAt 9:00 p.m. on January 2, 2018, an employee was providing in-home healthcare to a 96 year old man with dementia. It was a 12 hour shift beginning at 9 am. Around 9 p.m., the relief caregiver knocked on the client's door and there was no answer. She got the backup key from the box and opened the door to find the employee on the floor in front of the client who was in a recliner. She called 911. The paramedics could not revive him, and he was declared dead. The Coroner's autopsy report found the cause of death was: Cardiomegaly, unknown etiology; and the Final Classification was death due to natural causes.

NarrativeOn November 7, 2017, an employee was working alone providing home healthcare. Two residents got into an argument and one said she was leaving and taking the children with her. She went down stairs to a relatives 1st floor apartment with the second resident following close behind. The employee was helping the women get away, when she was stabbed in the heart and died shortly thereafter.
