NarrativeOn August 3, 2017 an employee was working alone and performing shingle repair on the upper 6:12 sloped and lower 4:12 sloped roofs of a building. The employee had used a two section 24' long metal Louisville extension ladder to access these roofs with the upper 12 foot section to access the lower roof from the ground and the lower 12 foot ladder section to access the upper roof from the lower roof. As the employee descended the ladder from the upper roof to the lower roof sections, the legs of the ladder slid out causing the employee to fall from the ladder onto the lower roof and then fall an additional 8 feet and 9 inches from the lower roof to the ground below. The employee died of spinal injuries caused by the fall.

NarrativeAt 10:15 a.m. on July 23, 2016, an employee was attempting to get a belay device un-struck from the face of the Owing-Spalding rappel on the Grand Teton Mountain. The employee was wearing a Mammut Climbing harness, using 9/16 8 foot synthetic flat strap runner as a positioning device. The employee was using two 20 meter climbing ropes to lower clients to the upper saddle floor in a belayed rappel. The employee was also wearing a climbing helmet. As the employee was working to un-stick the rappel device, the employee either unhooked his positioning device, or the positioning device broke, or the positioning device knot untied resulting in employee falling 120 vertical feet to the floor of the upper saddle. The employee then tumbled over the edge of Valhalla Canyon on the east face falling approximately another 2280 feet down the mountain side. The employee was killed and received very traumatic injury's to the entire body.
The accident investigation below was opened on Wednesday, June 15th 2016 by OSHA representitives in FREDERICKTOWN, MI which had occured previously at Madison County Sheltered Workshop. A had been injured when the fall on same level due to slipping

NarrativeAn employee had been doing landscaping outside and was walking when his right foot slipped forward, causing his left leg to buckle under him. He fell to the ground and landed on his left leg, breaking the fibula and tibia and requiring surgery.
