NarrativeOn the morning of June 21, 2019, Employee #1 and three coworkers, employed by a construction framing company, were preparing for roofing work at a two-story residential structure. They were on the second floor and assisting a crane operator who was landing plywood supplies. Employee #1 was directing the crane operator when a bolt came loose from the crane's hook and medicine ball. Twenty-four pieces of plywood, each measuring 8 feet long by four feet wide, fell on Employee #1. Emergency services were called and transported Employee #1 to the trauma center, where he was diagnosed with head and leg injuries. At approximately 11:30 a.m., Employee #1 underwent surgery. He was pronounced dead at approximately 12:30 p.m.

NarrativeOn January 29, 2019, an employee was unloading 2 x 10 boards from the forklift's tines and had grabbed three boards at once. The employee was pulled off balance, fell to the ground, and was struck in the head by one of the boards. Emergency Services worked on the employee for at least 45 minutes before attempting to transfer the employee to an airlift service for transportation to a hospital; however, the employee died before the transfer could be finalized.

NarrativeAt 9:30 a.m. on January 24, 2019, an employee was cutting a 2 foot x 2 foot HAVC chase in a wooden floor with a reciprocating saw and fell from the ladder he was using. The employee struck his head on the concrete floor and was killed.
