NarrativeAt 11:19 a.m. on March 21, 2019, an employee was rolling up a pressure hose. The motor of the hose was not shut off. As a result, the hose kicked back while the employee was rolling it, causing the its metal fitting to strike the employee. The employee was killed.

NarrativeAt 9:30 a.m. on Dec 21, 2018, an employee was prepping for the thermal insulation application by covering areas to be protected with plastic at a residential construction site. The employee was on the third floor of the home in front of an elevator shaft and had opened the interior door to the elevator to cover it with plastic. He fell into the elevator shaft and fell down approximately 20 feet to the ground. The employee was transported to the hospital via ambulance and died enroute.

NarrativeAt 3:15 p.m. on December 2, 2018, an employee was conducting spray foam installation activities. It appeares that the employee may have leaned against a guard railing that was not adequate in strength and design to support the employees weight. The guard rail became detached from the structure causing the employee to fall to the ground below. The employee was killed.
