NarrativeAt 5:34 p.m. on April 22, 2019, an employee was pulling the hoist cable of a Soilmec SR-30 Hydraulic Rotary Rig, with a pair of shakeout hook spreaders attached to the weight ball hook, to hook up a large metal pipe sleeve. The employee was killed when he pulled the hoist cable into a 13kV overhead high voltage power line and was electrocuted.
The accident investigation below was opened on Wednesday, April 24th 2019 by OSHA representitives in Henderson, KY which had occured previously at Allstate Tower, Inc..
