NarrativeAt 12:20 p.m. on February 16, 2019, an employee was moving equipment and allocating space for musicians to put down their instruments and personal belonging. The employee had driven a Genie GS-2632 electric scissor lift from inside the Stage Room out of the building doors and parked it alongside an outdoor air conditioning unit. The employee used a black poly tarp to cover the upper part of the scissor lift and was stepping down from the lift when his feet slipped and fell on top of the unit. The employee lacerated his upper left thigh and Emergency Services responded. The employee was transported to Community Hospital of Monterey Bay, where he was admitted for treatment.

NarrativeAt approximately 9:44 p.m. on November 2, 2017, Employee #1 was dismantling the set on a theatrical stage following a performance. Employee #1 was feeding rigging chain into a motor box approximately 12 inches from the edge of the stage. Employee #1 fell 5 feet to the concrete floor below the stage. Another employee contacted emergency services. Employee #1 was transported to a medical center. Employee #1 sustained a fractured skull, fractured collarbone, and 8 fractured ribs. Employee #1 died from the injuries 2 days later.
