NarrativeAt 8:45 a.m. on February 14, 2019, an employee was working from a Genie personnel lift (model number: PL-24PDC; serial number: 1480-1382) to change out light bulbs that were approximately 19 feet above the ground. The lift did not have outriggers installed. During work, the lift overturned and the employee fell, striking his head on the ground. The employee was killed as a result of the head injury.
OSHA investigators arrived in Houston, TX on Tuesday, December 27th 2016 and began an accident investigation after learning of an incident at Chancellors Family Center which had occured on Friday, December 23rd 2016. NarrativeAt 12:15 p.m. on December 23, 2016, and employee was preparing to remove a ligh t fixture and replace a lighting ballast for a fluorescent lighting system. Th e employee fell from a ladder, struck the concrete floor and sustained internal fracture/bleeding. The employee went into a coma and died.
