NarrativeAt 8:45 p.m. on October 28, 2017, an employee was walking up the gangway onto the fishing vessel. The employee appeared to have stopped at the top of the gangway and was in the process of coming off the gangway onto the steps when he fell overboard between the vessel and the pier and drowned. The employee had a large head wound and was unresponsive when removed from the water. A stanchion on the gangway was not secure and there was no safety net provided between the vessel and the pier. The employee fell from the top of the gangway, hitting his head either on the side of the vessel or the pier and then into the water.
OSHA investigators arrived in Ketchikan, AK on Wednesday, November 2nd 2016 and began an accident investigation which had occured previously at E.C. Phillips & Son, Inc. A 41 year old man working as a Fisher was killed on the job when the employee was crushed and killed when caught in ice machine

NarrativeAt 1:45 p.m. on November 1, 2016, an employee was clearing ice from inside an i ce making machine. The ice machine engaged and crushed the employee. The emplo yee was killed from asphyxiation.
OSHA investigators arrived in Honolulu, HI on Friday, May 4th 2012 and began an accident investigation which had occured previously at David Vo Fishing Llc. A 28 year old man working as a Fisher was killed on the job when the employee died from heart attack

NarrativeAt approximately 9:30 p.m. on May 3, 2012, Employee #1 was working on a fishing boat. As he exited the boat, he lost consciousness and collapse. Employee #1 was transported to a medical center, where he died later on due to a heart attack.
