NarrativeAt 3:45 p.m. on March 16, 2015, an employee operating a Cat Challenger 865C tractor, pulling a hydraulic plow system (Wilcox Agri-Products) turning up the soil on a parcel of land. The employee was found outside the cab of a Caterpillar Challenger MT865C crawler tractor approximately 150 yards behind the tractor and equipment face down, and deceased. The employee sustained a blunt force injury on the right hip (8x2 inch abrasion). The employee was crushed and killed as the Caterpillar Challenger MT865C Crawler tractor ran over his lower extremities.
The accident investigation below was opened on Wednesday, February 11th 2015 by OSHA representitives in CHILDRESS, TE after learning of an incident at Farmers Cooperative Gin which had occured on the same day. A had been injured when the caught in running equipment or machinery during regular operation

NarrativeOn or about February 11, 2015 at 3:20pm an employee suffered a broken left shoulder and hurt his left leg while working in the Press Area. He was caught in between the bag and a cotton bale while trying to put on a bag. The machine turned on automatically and pushed the cotton into the bag.
