NarrativeAt 9:30 a.m. on September 30, 2017, Employee #1 and his brother were working on a truck in the employer's shop. The employee and his brother lifted the back end of the truck and placed tires under it to support the truck. They then lifted the front end, using a forklift that belonged to the employer. Employee #1 got under the truck and was working on the motor mounts. The forklift rolled forward, pushing the truck off of the tires that were supporting the back end of the truck. The truck fell off of the forklift, crushing and killing Employee #1.
The accident investigation below was opened on Monday, September 11th 2017 by OSHA representitives in Mobile, AL after learning of an incident at Flowerwood Nursery, Inc. which had occured on the same day. A 59 year old man working as a Construction Laborer was killed on the job when the employee was buried when trench wall collapsed and was killed

NarrativeOn September 11, 2017, an employee was holding the end of a 20 foot long 8 inch diameter water pipe that was being positioned onto another water line by a coworker inside a trench. The employee was pinned against the trench wall when one side of the trench caved in and buried the employee up to his mouth. The coworker attempted to dig the employee out and Rescue Services took over once they arrived on scene. The employee suffered internal injuries and died approximately 3 to 5 hours later.
OSHA investigators arrived in Gridley, CA on Saturday, July 8th 2017 and began an accident investigation after learning of an incident at Agromillora California which had occured on Friday, July 7th 2017. A 58 year old Agricultural had been injured when the employee fainted and fell to the ground during hot weather

NarrativeAt 1:15 p.m. on July 7, 2017, an employee was sorting grafted trees for distribution to customers inside a greenhouse during a hot day. The employee became dizzy and fainted, falling to the ground. No hospitalization was required.
