NarrativeOn May 24, 2019, a heavy equipment operator was using a track hoe to move a tree when it fell off the excavator bucket and broke the glass on the operator's cab. The employee was killed when the tree struck the controls of the excavator causing it to move forward into the tree.

NarrativeAt 3:00 p.m. on October 24, 2018, an employee was driving a Hyster H100FT sit-down forklift and was unloading material from a metal basket into a trash compactor when the basket fell off the forks into the compactor. The employee dismounted the forklift truck with the forks raised and entered the front of the trash compactor. The employee was pinned in the compactor when the forklift rolled forward and trapped him between the forklift's mast and compactor. The employee was discovered on October 25, 2018, and was transported to the local hospital, where he was pronounced dead.

NarrativeAt 3:45 p.m. on December 4, 2017, an employee was climbing a scaffold ladder and accessing the scaffold platform. The employee grabbed onto the guardrail to climb underneath it when the rail came undone and the employee fell backwards from the scaffold. The employee sustained a skull fracture and was killed.
